Clinical charting

Dental charting built for how dentists actually examine.

Tooth-by-tooth charting with five surfaces per tooth, full periodontal charting, treatment overlays, and copy-from-prior. All in the browser — nothing to install, nothing to lose.

Tooth-by-tooth dental chart, five surfaces per tooth.

Tap a surface, pick a condition. Cavity, filling, crown, root canal, missing, implant — every standard finding is one tap, and every previous finding is right there for context. The chart re-renders instantly so the dentist stays in the patient, not in the software.

  • 5 surfaces per tooth (mesial, distal, buccal, lingual, occlusal) charted independently.
  • Standard condition palette: cavity, filling, crown, root canal, extraction, implant, missing, mobile, fractured.
  • Tooth numbering systems: Universal, FDI (ISO), Palmer — switchable per clinic.
  • Mixed dentition (deciduous + permanent) supported for pediatric clinics.

Full periodontal charting with copy-from-prior.

Pocket depths, recession, bleeding-on-probing, suppuration, mobility, furcation — all per-tooth, per-site. Copy-from-prior pulls the last exam into the new one so the hygienist only edits what changed, cutting exam time in half.

  • Pocket depths × 6 sites per tooth (3 buccal, 3 lingual).
  • Recession, BOP, suppuration, mobility, furcation tracked per tooth.
  • Copy-from-prior — last exam pre-fills, edit only what's new.
  • Trend view shows how each tooth changed across visits.

Treatment overlays and visit-mode.

Charting connects to treatment plans and the active visit. Mark a tooth treated and the linked plan stage advances; complete a visit and the chart snapshots so today's findings stay in history. Visit Mode surfaces only what matters today.

  • Per-tooth treatment overlay shows planned vs completed work.
  • Tooth → stage → invoice linkage; one click traces it end to end.
  • Visit Mode hides everything except today's planned work — fewer mistakes.
  • Locked by default, unlocked on Edit — guardrail against accidental changes.

Works in the browser, on the chair, on a tablet.

No installer, no plugin, no proprietary file format. Charting renders on a phone, tablet, or laptop with the same speed. Auto-save means a dead battery or browser tab close doesn't lose the exam.

Common questions

What clinics ask about this.

Which tooth numbering system does it use?

All three: Universal, FDI (ISO), and Palmer. The clinic picks one default; individual dentists can switch their view per session without affecting stored data.

Can the hygienist do the periodontal exam on a tablet?

Yes. The periodontal chart is touch-first — tap a site, swipe to next, pocket depths enter in seconds. Copy-from-prior pulls forward the last exam so the hygienist only updates what changed.

What about pediatric (deciduous) teeth?

Mixed dentition is supported. The chart auto-switches between deciduous, mixed, and permanent layouts based on the patient's age (and can be overridden manually).

Does it integrate with X-rays?

Per-tooth findings can reference attached X-ray images. The patient record stores X-rays per tooth or per quadrant; click a tooth on the chart to see linked images.

Can we print the chart for paper records?

Yes. Print and PDF export are available for the dental chart and the periodontal chart, formatted for clinic records or patient handouts.
